Family Fun Day - Registration Form 

Programme Information

Register now for our Family Fun Day on Tuesday 28th July, from 11:30am to 2:00pm.


In partnership with Fountains Family Hub, we are delighted to be hosting a Family Fun Day for parents and carers with children aged 0–5 years. Older siblings are also very welcome to attend, and there will be a selection of activities available for them to enjoy.


The day will be packed with fun, games, and engaging activities delivered by Everton in the Community and the Fountains Family Hub team. It will be a fantastic opportunity for families to play, learn, and spend quality time together in a friendly and welcoming environment.


We look forward to welcoming you and your family for a fun-filled day!

 

Please note that photographs will be taken throughout the Family Fun Day for promotional and publicity purposes. While we will make every effort to respect individual preferences, we cannot guarantee that attendees will not appear in wider group or event photographs.


If you do not wish for close-up photographs to be taken of you or your child, wristbands will be available on the day to help our photographers identify those individuals and ensure that no individual photographs are taken of them.

 • I authorise the member of staff that has a valid emergency aid certificate on site to administer any emergency medical treatment, including signing a consent form on my behalf if required by the medical services

 • I acknowledge that staff are only responsible for participants whilst they are attending the session . They are however free to come and go as they please during the sessions, and staff cannot be held responsible for their welfare once they have left the site

 • I acknowledge and accept that the football club or respective servants shall not have any liability in respect of any loss or damage to persons or property whilst in attendance on the applied sessions, and that members of staff reserve the right to refuse admission
 If this form has not been completed, participation may be refused.
